My thinking is that they made small amounts of fake blood, put it into small baggies, and then put the baggies in front of bottlecaps (or something similar) with small amounts of a low-yield explosive wired to a remote trigger. They then tape it to the wearer's body. When the trigger goes off, the explosive detonates, which causes a rapid expansion of hot gases that causes the fake blood to spatter as though the wearer has just gotten shot. I did once know for sure how they did this; I just haven't thought about it in a while and have since forgotten. I'm also sure there's some company out there that takes the human error of doing something wrong out of the picture by mass-producing a "safe" version of these things (even though even a prop can kill; for instance, did you know that a gun firing blanks can kill if it's fired too close to a person?).
