Just how realistic do want to get is the question you have to answer.
I've been visiting a site with a system based on the US Army's Robin Sage Special Forces exercise and it gets interesting, but it's not for your average game.
They use a deck of playing cards, levels of player and that sort of thing. You even draw a toughness card which tells the medic how strong you are (how much pain you can tolerate). If the medic can draw a card higher than your toughness card, then you survive and can be treated.
This system also uses a lot of other things that average games don't. You start as a basic rifleman, then advance. You can also have a specialty, such as medic, engineer, EOD, etc.
